Primary Duty: | Assesses and evaluates therapeutic rehabilitation/functional status, and participation in the development of the total plan of care. Initiates physical therapy program, instructs patient family/caregiver and/or personnel in the home therapy program. Instructs goals of the physical therapy program; provides a written home program. | ||||||||

Education Experience: |
Graduate of an
Physical Therapy curriculum approved by:
Minimum of one (1) year of appropriate clinical experience required. Home Health Care experience preferred. |

| Skills and Abilities: | Demonstrates good clinical skills in the treatment of neuromuscular and musculoskeletal problems. Possesses good oral and written communication skills. Possesses good organizational skills and willingness to work within a team concept. Must be a licensed driver with an insured automobile in good working order. | |||||||||
| Job Summary: | Organizes and conducts medically prescribed therapy program for clients involving exercise, massage, heat, water, light and electricity. Applies diagnostic and prognostic muscle, nerve, joint and functional ability tests. Directs and aids client in active and passive exercises. Evaluates, records and reports client progress. | |||||||||
