Primary Duty: | Assesses social and emotional factors relate to the patient's illness, need for adjustment to care, and response to treatment. Provides community resource planning, acquisition, linkage and discharge planning. | ||||||||

Education Experience: |
Manager's degree in Social Work (MSW) from a school of social work accredited by the Council on Social Work Education; qualified by the State of Michigan for licensure as a Certified Social Worker (CSW). One year of social work experience (post master's) in a health care or related setting. | |||||||||
| Skills and Abilities: | Knowledge of social work standards of practice and ethics. Experience and knowledge of community resources and counseling techniques. Strong interpersonal and communication skills with an ability to work within the team concept. Understanding of role to social work in home health care and reimbursement guidelines. | |||||||||
| Job Summary: | Identifies & analyses the social/emotional factors underlying client illness and communication these factors to the health team. Assists patients and their families in understanding & accepting treatment to permanent and temporary effects of illness. | |||||||||
