Primary Duty: | Assists patients in the performance of their activities of daily living and patient's personal hygiene. Reports changes in patient's status to case manager. | ||||||||

Education Experience: |
High School diploma or GED Equivalent preferred. Has successfully completed an approved Home Health Aide training program that meets the requirements of COP484.36(a) and competency evaluation program that meets COP484.36(b). Minimum one (1) year as a Nurse Aide or Home Health Aide preferred. | |||||||||
| Skills and Abilities: | Must be 18 years of age. Has the ability to read, follow written instructions and document care given. Demonstrates empathy for the needs of the patients. Must be a licensed driver with an insured automobile. | |||||||||
| Job Summary: | Provides personal care according to a written plan of care developed by the Case Manager. Assists with light housekeeping, meal preparation and other duties as outlined in the treatment plan. Reports patient status and significant changes regularly to Case Manager. | |||||||||
